I acknowledge that should this application be approved, any required financial/banking information/documentation required for payables will be provided upon request.

I acknowledge that our company will follow the HSE standards, practices, and requirements outlined within the Freight North Logistics and/or customer HSE Program(s), whichever is more stringent.

1. No Double Brokering: The applicant carrier agrees not to engage in double brokering. If it is discovered that a load has been double-brokered, payment will be withheld.

2. Insurance and Registration: All power units assigned to the loads must be insured and registered to the applicant carrier.

3. No Third Party or Subsidiary Companies: The applicant carrier agrees not to utilize any third-party or subsidiary companies in fulfilling the terms of this agreement.

I have read and understood the terms and conditions of the Freight North Logistics' Sub‐contractor Pre‐qualification Requirements. The information provided in this package to Freight North Logistics is accurate and true to the fullest extent possible.
